Annual enrollment fee is only $325 for a single individual. Each additional family member added to the plan is $225. If you have any questions, please feel free to contact our office manager.
What Is Covered?
A) 2 X Comprehensive exam, All X-Rays and intra oral pictures needed for diagnosis, cleaning ANF FL Where it applies.
Keep in mind ADA recommends check ups and cleanings every 6 months for optimal dental health
B) In-house complimentary consultation by other specialists.
Additional Benefits
20% off fillings, pedo crowns, extractions.
There are no annual limits. *** Does not apply to orthodontic treatment.
Terms: You can renew your enrollment automatically by providing us with a credit card on file
The enrollment fee is non-refundable once the initial treatment started.
If you have been diagnosed with periodontal disease, your three to four months recall visits are not covered by the enrollment fee, but the 20% deductible applies.
This insurance plan does not apply to any other advertised specials.
